The Humber Coast and Vale Sustainability and Transformation Plan (STP) is the first stage in a programme of work that was started in April of this year, by a partnership of local authorities, NHS comissioners and providers, as well as other community based organisations.
STPs are about making practical changes in the way they deliver services for people and communities. Their vision is for everyone living in Humber Coast and Vale to start well, live well and age well by helping patients to manage their own care effectively, reduce dependance on hospitals and use resources more effectively.

Over the coming months they will be building on the engagement they have carried out over the past two years, and will be working with Healthwatch and other voluntary sector partners to ensure that they have sought out and heard views from a wide range of communities. The ideas put forward from these groups will then be built into their future plans.
The STP will continue to provide updates and information about engagement opportunities through Accord, stakeholder and community networks
