Having access to the right information, advice and support can help people feel like themselves again more quickly and avoid them having a setback.<\/p>\n<p>North East Lincolnshire Clinical Commissioning Group (NEL CCG) and Northern Lincolnshire and Goole NHS Foundation Trust (NLaG) want to make sure everyone gets the information they need to help them continue on the road to recovery. To help this happen, we need people who have come out of hospital in the past 12 months to tell us about what it was like when they were discharged, what sort of information they were sent home with and how they are doing now.<\/p>\n<p>\u201cWhile some wards provide patients with lots of information about things like wound care, managing pain or food and drink advice, we know this doesn\u2019t happen everywhere or for every patient,\u201d explains Dr Rakesh Pathak, a GP representative with NEL CCG in Grimsby.<\/p>\n<p>\u201cExcellent discharge planning can dramatically improve the outcome for patients. It can also make life a little easier for them at a difficult time if they have the right information to hand about benefits, sick notes and support at home that might be available to them. We want to make sure this is something that all local people can expect wherever they have had their treatment.\u201d<\/p>\n<p>\u201cWhile we know hundreds of our patients leave hospital every year fully \u201cin the know\u201d about after-care and the steps they can take to speed up their recovery, we want to make sure this is standard across all of the wards and whether there is anything else we can do to make the transition from hospital to home smoother,\u201d said Karen Fanthorpe, Deputy Chief Operating Officer for NLaG.<\/p>\n<p>We want to hear about people who have left hospital in the past 12 months and would like to hear from them or their carer. This survey isn\u2019t about people\u2019s experiences during their stay in hospital, we want to hear what happened AFTER their discharge and we will ask questions about the written information they were given and what other advice they were offered.
